WI Act 172 - Concussion Information

*** IT IS EXTREMELY IMPORTANT THAT YOU READ THE FOLLOWING IN ITS ENTIRETY ***

On April 2, 2012 the Wisconsin Legislature passed Act 172, a law requiring that all athletes, parents, and coaches participating in youth or High School athletic activity acknowledge by signature that they have been informed about the effects and symptoms of concussion before participating in the sport.  
 
STATE LAW REQUIRES THE FOLLOWING BEFORE YOUR ATHLETE WILL BE PERMITTED TO PARTICIPATE IN ANY LACROSSE ACTIVITIES
 

1) You and your athlete must review the concussion information sheets available on the Wisconsin Department of Public Instruction website.
 
2) Having reviewed this information, you both you and your athlete are required to sign and return the Concussion Agreement (see below) to Hot4Lax before he/she will be permitted to participate in any lacrosse activities.
